Registrants are required to maintain records of their CPD participation for 5 years, which includes an activity tracking sheet and supporting documentation of completed CPD activities.
Activity Tracking Sheet
Your activity tracking sheet documents your progress towards the CPD requirements during an assigned two-year cycle and is used by the QA Committee to verify your participation if audited. Your tracking sheet should include the following information:
- Cycle dates
- Activity completion date
- Activity category (Section A, B, or C)
- Content category, if applicable (Ethics, EDI, and/or Supervision)
- Brief activity description, including: the name of the activity organizer/speaker/presenter, name(s) of colleagues involved, and details of the topics or content discussed.
- Brief reflection on how the activity enhanced your knowledge/skills or supported the achievement of objectives set out in your self-assessment
For each QA cycle, use a new tracking sheet.

SUPPORTING DOCUMENTATION
You’re required to keep supporting documentation to confirm your participation in the CPD activities you’ve recorded. Examples of suitable supporting documentation include:
- Certificates of completion or CE certificates
- E-mail correspondence or a letter confirming attendance
- Registration confirmation
- Course syllabi
- Receipts for paid event attendance or materials
- Event agendas, handouts, or presentation slides
- Dated activity or self-study notes
If documentation is not available or provided for an activity, your tracking sheet should include sufficient detail in the activity description to confirm completion. Details should include:
- Name/title of the event, course, program, etc.
- Name of the activity organizer, provider, speaker or association
- Citation or weblink for any self-study resources (e.g., journal articles, books, etc.)
- Names of any peers or colleagues involved (e.g., supervisor/supervisee, consulting practitioner, etc.)
